Portraits for Humanity Submission Upon request, after filling out this form, clients will be selected to receive a discount of $250 for a realistic black and grey portrait. This is a grassroots project dear to my heart that began as a vision I had in 2017 connecting art & activism during a time that felt increasingly racist, misogynistic, homophobic, transphobic & generally unsafe. The appreciation I had & still have for my diverse clientele inspired me to believe in this project even when it felt kind of out there. “We can’t change the world unless we change ourselves” -Biggie •What is your name? •Where are you from? •Who is the portrait of? •Where do you want the portrait? •How did you hear about this project? •What cause/random act of kindness do you wish to contribute to with the $50? (Can list multiple causes or creative ideas. This can be a cause you are passionate about or a spontaneous act of generosity. Feel free to think outside of the box!) •Is this your first tattoo? •What is your pain tolerance level from 1-10 (1 can handle a lot of pain/can sit through a six hr tattoo. 10 being you have a very difficult time enduring pain and need to do multiple sessions). •Feel free to include any additional details you would like me to know about why you would like to be selected for this project.   •Please include any and all links to charitable events, organizations, causes or specific people you’d like us to network with! Cancelation Policy

The deposit is non-refundable. In order to keep your appointment you must show up to your scheduled date and time. New appointment new deposit. No call no shows will result in an additional deposit charge to reschedule.
